Assessments and Evaluations

Substance Abuse Evaluation

  • Intake
  • SASSI – 3
  • SOCRATES – A, and
  • SOCRATES – D
  • DSM – IV Questionaire

This evaluation generates a DSM – IV Axis I diagnosis of client’s level of involvement in substance use/abuse, and, depending on the diagnosis, recommendations for treatment.

Time:  2 – 3 hours
Cost:  $250.00

 

Mental Health Assessment

  • Intake
  • Differential Diagnosis
  • In-depth interview

This assessment will help to guide treatment recommendations and determine whether – or not – the individual needs a full mental health evaluation and at what level.

Time required:  2 – 3 hours
Cost: $130.00

(If it is determined that a Mental Health Evaluation is needed, the cost of the Assessment would be applied to the Evaluation.)

Mental Health Evaluation – I

If an assessment was not completed, the Intake and Differential Diagnosis will be done. Followed by:

  • SCL – 90R
  • SASSI -3
  • Beck Depression Inventory
  • Beck Anxiety Inventory
  • In-depth interview

This evaluation would generate a Multiaxial diagnosis and treatment recommendations.
Cost:  $350.00; or $220.00 – if a Mental Health Assessment was completed.

Mental Health Evaluation – II

For clients with a low Global Assessment of Functioning (GAF), which would be revealed in the Differential Diagnosis, and/or exhibiting behaviors indicative of a severe and persistent Axis I diagnosis:

  • MCMI – this instrument would verify the Axis I and Axis II diagnosis arrived at through the Mental Health Evaluation I and looks at the interaction between Axis I and Axis II. The interpretation of the MCMI would be reviewed by a consultant with a PhD in Psychology.
    If indicated:
  • ASEBA – through this instrument collateral information is gathered from friends and family, allowing for a systemic ‘picture’ of the client’s functioning.

This process generates information that will provide a more in-depth and systemic view of the client’s mental health status and his/her environmental challenges and supports. It will provide a basis for recommending a treatment plan with the best possibility of a desirable outcome.

Time:  Minimum of 5 hours (can be considerably longer if ASEBA is used)
Cost:  $500.00 to $1000.00 – dependent upon whether the ASEBA is used. If following an assessment, the cost of the assessment would be deducted.

Anger Management Evaluation

  • Intake
  • Differential Diagnosis
  • SASSI - 3
  • Beck Depression Inventory
  • Beck Anxiety Inventory
  • Family History Re:  Anger
  • “Do You…….” Questionaire

This evlauation generates a DSM – Multiaxial Diagnosis of client’s impulse and anger control and emotional regulation.  Recommendations for treatment are based on the findings.

Time:  2 – 3 hours
Cost:  $350.00
